Food Concessions Application and Agreement (Check)

As a participant of the 2020 North Canton Main Street Festival, I fully understand and agree to the following:

I recognize and acknowledge that I assume full risk of any injury, property damage, or loss which I may sustain as a result of my participation in any and all activities connected with or associated with my participation in the Festival. Furthermore, I understand that I must carry my own insurance.

I agree to waive and relinquish all claims I may have against the North Canton Chamber of Commerce, the City of North Canton, Know Your Roots, and all associated sponsors and staff of the Festival and agents thereof from any and all claims other parties may have resulting from injuries, damage, or loss caused by, arising out of, connected with, or in any away associated with the activities of the Festival. I further agree to provide a certificate of insurance naming the North Canton Area Chamber of Commerce as an additional insured on the policy by the application deadline of Wednesday, August 26, 2020. (Failure to do so will delay authorization for you to participate in the festival.)

I agree to provide a specified list of all menu items I plan to sell, and I acknowledge that I may not be permitted to sell each of the items I submit. I further agree to sell only the accepted items at their accepted prices.

I understand that I must comply with all Stark County Health Department regulations pertaining to food sales at special events. I understand that failure to comply with SCHD regulations may result in being asked to leave the Festival, forfeiting all fees. I understand that all permits must be purchased and be available to display prior to set-up, that no one will be permitted to set up without the temporary food license, and that the SCHD will be on site to inspect all units.

I agree to pay the necessary fees set forth in this application. I understand that if I am not accepted into the North Canton Main Street Festival, I will receive a refund of my Food Vendor Fee.

I agree to be present during the date/hours specified on this application.

I understand that I am solely responsible for the conduct and maintenance of my operation and agree to act at all times in a professional manner.

Per fire code, any unit using electricity for cooking or warming food requires a fire extinguisher. North Canton Fire Department personnel can help answer any questions. Any revisions or updates to the Ohio Code will be shared with you prior to August. Additionally, any vendor using a generator must be equipped with a 5 pound fire extinguisher.

I have read through the application and agree to abide by all rules and regulations established by state and local law, and those rules and regulations established by the North Canton Area Chamber of Commerce and the City of North Canton.
